Purpose. The standard format for an incident action plan at a Level 2 or Level 3 incident, recording the situation, objectives, strategies, structure, resources, communications and safety for an operational period.

1.How to use this template

The incident controller, or the planning officer at a Level 3 incident, must prepare an incident action plan for each operational period. The plan must be briefed to every sector commander and functional officer at the start of the period and a copy sent to the communications centre and recorded against the incident in the Incident and Dispatch Log.

2.Situation

Record what has happened, what is happening now, the current and forecast weather, the threats to life, property and environment, and the predicted behaviour of the fire, flood or hazard over the operational period. Use the intelligence gathered by the planning function and state the confidence in each element.

3.Objectives and strategies

State the incident objectives in priority order, beginning with the safety of members and the public. For each objective state the strategy and the tactics assigned to each sector, and the trigger points at which tactics change or crews withdraw.

4.Structure and resources

Record the incident management structure with names and call signs for the incident controller, safety officer, sector commanders and functional officers, the resources assigned to each sector, the staging area and the resources requested but not yet arrived.

5.Communications and safety

Record the radio channels for command and each sector, the reporting times, the medical plan, the safety message for the period including the key hazards and controls, the fatigue management arrangements and the welfare facilities.

6.Approval

The incident controller must sign the plan, record the operational period it covers and the time it was issued, and record the time of the briefing.
